Commercial Loaner Agreement
Please complete the Loaner Agreement with Company Name, choice of loaner period and authorized signer information. A signature is not needed at this time. Then forward the form to Treadwell Corporation. Treadwell will complete the form after confirming delivery charges, if any, confirming delivery and training date, and will provide the device serial number. The form with completed information will be returned to the Company for signature and return with payment to Treadwell Corporation at the address at the bottom of the Loaner Agreement or via PayPal or other payment options available here.
Your representatie will be contacted to confirm a convenient delivery date, time for delivery and instruction on use within 7 business days of receipt of both your agreement and payment.
The Commercial Loaner Program is currently available in Eastern North Carolina area only.
